Probably because of no soul Sam thing in season 6. Angel was too embarrassed and scared to show up before Moose-no-soul-and-angry-bigboi
Well, Sam does have some significant things going against him where Cas is concerned. He is (1) the specially prepared vessel for Lucifer (not his fault, but still) who (2) willingly started drinking demon blood, became addicted to it, and released Lucifer from the cage. There are others, but these are the two big things I think are more significant.
Now that's not say that Cas and Dean both don't have their own faults, but these are _the_ first dealings Cas had with Sam from the moment he got involved with the Winchesters, and first impressions are powerful. That's not to say they can't be overcome---and Cas has overcome them to a very significant degree---but in the recesses of his memory Sam is still that demon-blood-guzzling vessel of Lucifer.
